The Form view offers a simple, clean form to create new records in a table. Users only see the visible fields, fill in the form and submit. Each form generates a shareable link: data entry happens without direct access to the table.
Do not confuse the form view with the external form. To edit existing records, use the External form column.
Prerequisite: the table first, the form second. The form does not create fields: it displays the fields of the table. First create the columns and set their options in the table (conditional visibility, conditional requirement, conditional read only), then switch the view to Form view: there you can still set each field's access to Read / Write, Read only or Invisible (see Restrict access to a column by view). Configure form comes last, for the layout and formatting.
